Credit Application

Thank you for your interest in becoming a dealer for Curtis Industries, LLC!

We invite you to download the Dealer Credit Application and complete with the required information to become a dealer of Curtis products and accessories for golf cars, UTV-powersports vehicles and compact tractors.

To become a dealer, we require all applicants to be a qualified OEM vehicle dealer for any of the various manufacturers we currently supply our products for.  These include compact tractor manufacturers such as John Deere and Kubota; UTV-powersports manufacturers such as Polaris and Yamaha as well as golf car manufacturers.  The second criteria is to be able to meet our opening order requirements consisting of common and replacement parts.  Lastly, we require our dealers to be able to provide an independent location for end-users to make purchases directly through a dealership affiliated with a vehicle manufacturer.   


If you meet all the criteria listed above, please return the application via fax to our Sales Department at 508.854.4912. You will receive acknowledgement to initiate the review process and incomplete information will be requested at that time.  Once the process is complete, you will be notified by our Sales Department with a dealer number along with to the terms and conditions of your account including ordering processing and payment terms.
